Paciente de 41 años que vemos por primera vez en 2015, con antecedentes de 2 cirugías por FUT y otras dos cirugías por FUE, 9 años atrás. En tratamiento con finasteride 0.5 mg/d y minoxidil. Su primer objetivo fue reparar la cicatriz posterior, por lo que se extrajeron 301 grafts (151 de cuero cabelludo y 150 de BHT de barba). Al año el paciente nos solicita si podemos retirar todos los grafts de la zona receptora, ya que considera que sus escasos injertos son antinaturales y muy evidentes, y utilizarlos para aumentar la cobertura de la cicatriz y reparar los white dots de sus cirugías por FUE. Se extraen 806 grafts de su receptora a 1,58 pelos/graft y se colocan en las zonas mencionadas. Desglose: I-398; II-354; III-50; IV-4. La dificultad de la extracción de unidades trasplantadas radica en que no tienen una anatomía normal, ni en profundidad ni en dirección, por lo que la trasección es mucho más frecuente. Hace falta mucha paciencia y experiencia. Al año (2018), nuevamente se intenta la extracción de los grafts remanentes en la zona receptora, esta vez 237 intertos a 1,16 pelos/grafts. Desglose: I-203; II-31; III-3. El paciente ya no toma medicación. A los 14 meses el paciente nos pide camuflar la parte inferior de la cicatriz (alopecia retrógrada). Se realiza pequeño retoque de 301 grafts (2,24 peños/graft; 675 pelos; desglose. I-35; II- 162; III- 100; IV -4). El video muestra el resultado de la receptora, limpia ya de injertos, y el estado de la donante con pelo largo, corto y rasurado justo antes de esta cuarta cirugía.
